Case Manager

Summary Description

Case managers are responsible for determining the needs of the member and coordinating services to ensure that needs are met. Further responsible for monitoring the implementation of the Plan of Care.  Coordinates multiple services for members through comprehensive assessment, care plan development, monitoring and follow-up activities, and referrals to other needed services. 

Accountabilities / Essential Functions

  • Comprehensive assessment and periodic reassessment of individual needs, to determine that need for any medical, education, social or other services.
  • Development (and periodic revision) of a specific care plan that is based on the information collected through the assessment and URAC/ specific Contract requirements.
  • Referral and related activities to help the eligible individual navigate benefits and obtain needed services
  • Monitoring and follow up activities as required by Level of Care and Contract deliverables
  • Document activities in computer system
  • Other duties as assigned

Qualifications

  • RN with active unrestricted license to practice in Contracted State or hold Compact State License.
  • Graduation from accredited school of Nursing
  • CCM preferred

Skills, Knowledge and Abilities

  • Good communication skills
  • Ability to work in a faced paced, changing environment
  • Must be able to work independently with minimal supervision
  • Must be a Team Player
  • Knowledge of case management methods, procedures and practices;
  • Ability to assess and reassess continuing member needs;
  • Ability to develop and implement member Plan and determine the services most appropriate to meet the assessed need(s);
  • Ability to monitor the implementation of the plan of care
  • Provide guidance to assist members in utilizing community services effectively and appropriately;
  • Must have experience interacting with individuals, service providers, family members, employers, other professionals, etc.
  • Knowledge of URAC standards

Experience    

  • RN with at least 5 years Clinical Experience
  • 2 years Managed Care experience
  • 1 year CM experience
