 Clinical CareCoordinator

The Clinical Care Coordinator will act as a patient advocateduring the appeals and complaint processes by providing Immediate Advocacybetween the patient and the facility to ensure that the patient has what isneeded. The Clinical Care Coordinator will work closely with the social workerswho are currently and who will continue to do Immediate Advocacy withbeneficiary callers. They will also serve as coordinators in theBeneficiary Healthcare Navigation Program to assist with coordination ofservices to individual beneficiaries. They will collaborate with the PFE HealthEducator for creating and presenting educational programs to staff and others,work closely with the BFCC Outreach Staff for communication, and helpcoordinate PFE activities within the BFCC office.


  • Perform direct beneficiary/family advocacy to resolve issues
  • Provide care coordination and navigation to beneficiaries in need of such services 
  • Coach beneficiary/family for positive interactions with the healthcare community.
  • Coordinate PFE activities within the office
  • Interact with providers/plans to improve PFE efforts
  • Provides education and resources to beneficiaries/families/providers
  • Work with data analytics team to identify opportunities for improvement, high-risk target populations, and evaluation.


  • RNs, LPNs, SW and experience w/Patient Navigation
  • Bachelor’sdegree in Nursing strongly preferred
  • Active,unrestricted with current State licensure.  
  • Four years progressively responsible experiencein case management, communications, and/or advocacy
  • Experience with project coordination
  • Knowledge and familiarity with publicsector health coverage systems, e.g. Medicare / Medicaid.
  • Knowledge of principles, ethics, andprecepts of care management service delivery.
  • Knowledge of customer serviceprinciples
  • Knowledge of regulatory andaccreditation standards.
  • Strong written and verbal communicationskills.
  • Strong telephonic interviewing skills.
  • Strong computer skills, e.g. caremanagement applications, Internet/Web, Microsoft Office (Word, Excel).
  • Strong prioritization andorganizational skills.
  • Ability to demonstrate purposeful andoutcomes directed critical thinking within daily operational activities.
  • Ability to receive verbal and writtenfeedback in a professional manner and implement performance and productivityimprovements as needed.
  • Ability to interpret real-time andhistorical information
