JOB TITLE:            PASRREvaluator for Tallahassee, Florida

REPORTS TO:      Director,Program or Director, Quality

Summary Description

The PASRR Evaluator is responsible for completion ofclinical evaluations and determinations of individuals referred by acute careor nursing facilities for a Level 2 pre-admission screening or resident review.These are individuals with a suspected or confirmed mental illness and/orintellectual disability or related condition and who are seeking admission to aMedicaid certified nursing facility, or who are currently residing in a nursingfacility whose physical and/or mental functioning has significantly changed.The PASRR Evaluator communicates with service providers, reviews availablemedical records, consults with family or other collateral sources, and interviewsthe patient.  The PASRR Evaluatorconducts a face-to-face assessment or reviews PASRR Level 1 documentation andprovides recommendations.  This positionhas fantastic growth potential. 


  • Ensures accurate PASRR evaluations and determinationsthrough accurate clinical assessment of the patient, relevant records andcollateral sources of individuals with suspected or confirmed mental illness todetermine the presence of a Serious Mental Illness (SMI), as defined by 42 CFR483.102(b)(1) and F.A.C. 59G-1.040(2)(m). 
  • Ensures accurate PASRR evaluations anddeterminations through accurate clinical assessment of the patient, relevantrecords and collateral sources of individuals with suspected or confirmedintellectual disability or related condition to confirm or rule out thepresence of an intellectual disability or related condition (ID/RC), as definedby 42 CFR 483.102(b)(3) and F.A.C. 59G-1.040(2)(b). 
  • Ensures accurate PASRR review completion bydetermining appropriate level of care based on the patient’s physical,behavioral and psychological functioning.
  • Ensures compliance with necessary workflowinclude re-review of records based upon input from QA Director and/or MedicalDirector. 
  • Respond to state inquiries regarding evaluationsconducted.
  • Attend and actively participate in staffmeetings and conduct case consultations/peer reviews/internal auditing asassigned.
  • Review current literature, regulations andresearch related to long term care, mental illness, medical conditions andgeriatrics.

  • Licensed Registered Nurse, Licensed ClinicalSocial Worker, Licensed Clinical Psychologist, Nurse Practitioner or LicensedMental Health Counselor. Licensed by the state of Florida.
  • Knowledge of Federal Regulation 42 CFR, Chapter483 and F.A.C. 59G-1.040.
  • Knowledge of psychiatric disorders as defined bythe most current Diagnostic and Statistical Manual in use in California.
  • Knowledge of standards of practice related todetermination of Level of Care.
  • Knowledge of standards of practice related tomental health, geriatric and other disorders typically requiring long term careplacement.
  • Knowledge and understanding of public sectorservices and supports.
  • Computer proficiency in Microsoft Excel, Wordand Outlook.
  • Ability to determine a clinical diagnosisaccounting for psychological and behavioral issues.
  • Ability to utilize computer equipment and webbased software to conduct work.
  • Ability to interact with various office staff asneeded to support necessary workflows.
  • Ability to interact with healthcareprofessionals, patients, their families and other supports.
  • Ability to provide relevant training to clinicalstaff, including new hires.
  • Ability to communicate effectively toindividuals and groups through spoken, written and electronic media.
  • Ability to attend to detail, effectivelyprioritize and execute tasks in a timely manner.
  • Assesses and analyzes psychological, emotional,behavioral and physical functioning.
  • Develops level of care recommendations basedupon clinical evaluations.


  • Minimum of 3 years’ experience in the assessmentand diagnosis of individuals with mental/psychiatric disorders, includingcomplex differential diagnoses that are related to substance use, medicalconditions, cognitive issues, and dementia.
  • Experience with geriatric population ispreferred but not required.
  • Supervisory experience is a plus.
  • Minimum of (1) year of experience in workingwith individuals with intellectual disability or related conditions.
