RN Case Manager - Geriatrics

Responsible for assessing, planning,implementing, monitoring and evaluating options and services to effect anappropriate, individualized plan for the beneficiary across the continuum ofcare.  Using independent judgment, the case manager utilizes clinicalknowledge and competence, communication skills, problem solving and conflictresolution to effectively ensure optimal client outcomes with consideration topayer requirements.  Also coordinates beneficiary needs related tocommunity resources, financial assistance programs, and long-term activity ofdaily living needs outside the benefit criteria of the medical insurancecoverage.  Will work effectively with all health care team membersinternally and externally. 

  • Manages care of beneficiaries through the health care system based on the individual needs of the beneficiary.
  • Uses independent judgment and discretion to address and proactively resolve barriers impeding the diagnostic or treatment progress. 
  • Interacts and collaborates with multidisciplinary care team, which includes physicians, nurses, case managers, pharmacists and social workers/educators to ensure beneficiary needs and preferences for health services/information is shared; educates beneficiaries about community resources/options; advocates on behalf of the beneficiary.
  • Establishes a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality outcomes.
  • Maintains open communication with all appropriate parties.
  • Maintains strict standards for client confidentiality and client related information.  Complies with all organizational, state and federal regulations and policies on confidentiality. 
  • Prepares documentation, status updates, event notifications and other documentation regarding beneficiaries in accordance with regulatory requirements and company policies and procedures.
  • Monitors case load to ensure all required documentation and entry of assessment results into web-based database are completed accurately and timely.
  • Pursues ongoing education, certification, and self-development to remain current with case management standards.

QUALIFICATIONS

  • Active, unrestricted RNlicensure and graduation from School of Nursing required; Bachelor Degree andCCM, preferred.  
  • Minimum 3 years clinicalexperience in acute care, home health care or combination - AND - 3 yearscase/utilization management or discharge planning experience
  • Excellent verbal andwritten communication skill
  • Ability to utilizecritical thinking and apply sound judgement and assessment skills for decisionmaking
  • Excellent interpersonalcommunication and negotiation skills
  • Strong analytical anddata management skills
  • Strong organizationaland time management skills and the capacity to prioritize multiple tasks
  • Current workingknowledge of utilization management, case management and managed care
  • Organized and efficientin prioritizing and managing assignments with minimal oversight and direction
  • Computer proficiency inMicrosoft Office and other software programs and the ability to enter andretrieve data from relevant computer systems
  • Ability to communicateeffectively and interact with beneficiaries and others involved with thebeneficiary in a professional and courteous manner. Strong focus on customerservice & customer relations.
  • Ability to handlesmultiple tasks and adjust to changing priorities.
