Socially Necessary Services Care Manager

Performs utilization management tasks, training and technical assistance for SNS providers in a concise and clear manner to maintain high customer satisfaction and aims of the program for the WV DHHR Bureau for Children and Families.  SNS providers range from biological parents to professional agencies requiring assistance with the authorization process.  Provides consultation regarding child welfare cases to BCF Policy/Management staff as well as DHHR field staff related to abuse/neglect and juvenile justice cases to ensure the appropriate services are being authorized.  Care Manager represents the contractor at public forums, conferences and meetings regarding the program and assists with further refinement of the model.

  • Review pended cases within 2 business days of receipt by utilizing the UM manual, interpretive guidelines, BCF policy and BCF procedures
  • Provide consultation regarding child welfare cases to BCF Policy/Management staff as well as DHHR field staff related to abuse/neglect and juvenile justice cases to ensure the appropriate services are being authorized
  • Conduct training and consultation to providers and BCF staff regarding SNS policies and procedures, consultation instruments based upon analysis of information obtained via authorization process and review of individual cases.
  • Represents the contractor at public forums, conferences and meetings regarding the program and assists with further refinement of the model
  • Will participate in monthly ASO Group Support meetings, quarterly all-staff meetings and other required meetings as needed 

QUALIFICATIONS

  • Master’s degree in social work, counseling or psychology from an accredited program
  • Current WV Social Work, WV Counseling or WV Psychology License in good standing with the corresponding board
  • Minimum 1-2 years experience in direct practice with children and families involved with the child welfare system
  • Demonstrated experience writing/evaluating service plans, leading multi-disciplinary  teams, providing educational/training to diverse populations and/or public speaking preferred and customer Service in which both verbal and written communication were required
  • Knowledge of:  WV DHHR Child Protective and Youth Services Policy; WV statute related to abuse, neglect and juvenile proceedings; Child Welfare Best Practice Standards; Supports and Services available across the state; and Health Information Privacy and Portability Act
  • Skills:  Utilize Word Perfect/MS Office and Web applications;       Communicate clearly through verbal and written means with providers with different levels of experience; Organize work time and tasks effectively to meet contract deliverables; Negotiation and de-escalation capabilities and the ability to learn the basic policy and procedures of each contract/be cross trained to provide support to other programs when needed
  • Ability:  Analyze/assess service requests to determine if eligibility, admission, continued stay and/or discharge criteria are met; Prioritize tasks effectively; Interpret utilization management, training/consulting scores and CareConnection® data; Evaluate provider case records against designated documentation and practice standards and Interpret RFP and program data to allow team to respond to announcements innovatively
