Supervisor, Case Management 

Responsible for the supervision and day-to-day activities of Case Management teams. The Supervisor will mentor, coach and train team members in the care management process to ensure quality and contract deliverables are met. Using independent judgment the Supervisor, utilizes clinical knowledge and competence, communication skills, problem solving and conflict resolution to effectively ensure optimal client outcomes with consideration to payor requirements. 


  • Supervise, mentor and coach assigned Case Management team in the management of members through the health or social care systems based on the individual’s needs, utilizing clinical knowledge, communication skills, and problem solving and conflict skills.
  • Assesses, evaluates and addresses work load; adjusts to meet the demands of the department and contract requirements
  • Identify orientation and continuous learning needs in collaboration with direct leadership, develops and implements plans to address those learning needs.
  • Responsible for quality monitoring activities such as identifying areas of improvement for individual team members.
  • Uses independent judgment and discretion to address, resolve, and process problems impeding the diagnostic or treatment progress.  Proactively identifies and resolves delays and obstacles.
  • Coordinate care to promote best practices requests
  • Participate in development, implementation and revision of guidelines and clinical review criteria
  • Utilize clinical expertise to achieve inter-rater reliability
  • Maintains open communication with all appropriate parties.
  • Maintains strict standards for client confidentially and client related information; complies with all organizational, state and federal regulations and policy on confidentially.
  • Remains current on care management documentation and completes reports on time.


  • Bachelor’s degree in Social Work or a related field from an accredited college
  • Clinical License strongly preferred
  • Minimum three (3) years' experience providing Case Management services
  • Minimum three (3) years' experience in the field of DD
  • Demonstrated previous workload/staff management experience
  • Reporting requirements for Adult and Child Protective Services and DDP incident Management Policy.
  • Knowledge of case management methods, procedures and practices;
  • Ability to assess and reassess continuing member need;
  • Ability to develop and implement member Plan and determine the services most appropriate to meet the assessed need(s);
  • Ability to monitor the implementation of the plan of care
  • Provide guidance to assist members in utilizing community services effectively and appropriately;
  • Promote members’ self-determination.
  • Must have valid driver’s license and private transportation
  • Must have experience interacting with individuals, service providers, family members, employers, other professionals, etc
  • Excellent verbal and written communication skills essential.
  • Ability to utilize critical thinking and apply sound clinical judgment and assessment skills for decision making.
  • Excellent interpersonal communication and negotiation skills required.
  • Strong analytical and data management skills.
  • Strong organizational and time management skills and the capacity to prioritize multiple tasks.

Entry Level pay for Supervisor, Case Workers is $65,000. At the time of offer, salaries will be based on several factors to include education and relevant experience.
