Team Lead, Case Manager – RN, LSW, LISW

Coordinates the activities of Case Management staff.  Conducts assessments of evaluations and determinations;  assures accuracy and timeliness of all evaluations and determinations.  Assesses, evaluates and addresses daily work load; adjust work schedules daily to meet the assigned work.  Analyzes productivity of staff. Meets and maintains all applicable State and Federal laws, rules, and regulation regarding implementing and administering.

Maintains current knowledge base related to processes and practices related to the assessment processes; functions as the initial resource to Case Managers regarding all assessment/determination process questions and/or concerns.

As necessary responsible for assessing, planning, implementing, monitoring and evaluating options and services to effect an appropriate, individualized plan for the client across the continuum of care. 


  • Current, valid and unrestricted Ohio RN w/one (1) year paid clinical experience in Home and Community Based Service - OR - 
  • Current, valid and unrestricted Ohio LSW or LISW w/one (1) year paid clinical experience in HCBS.
  • Current, valid and unrestricted Ohio drivers' license and reliable transportation
  • Expertise and experience in gerontology, pediatrics, developmental disabilities, chronic diagnoses, education, child development, vocational services, substance abuse, behavioral health, transition planning or relocation, or independent living skills STRONGLY PREFERRED.
  • Workload management experience required
  • Ability to make face-to-face visit, phone conversation, email exchange, or other electronic communication with an individual that ensures and results in, the exchange of information between the case manager and the individual
  • Ability to multi-task and prioritize with variable and sometimes conflicting deadlines; superior attention to detail and demonstrated ability in decision-making.
  • Demonstrated initiative and judgment in performance of job responsibilities, while maintaining professionalism, flexibility and dependability under pressure.
  • Strong communication (written/verbal), interpersonal, organizational, time management and communication skills with a strong focus on customer service, including  building and maintaining relationships with internal/external customers and facilitating meetings.
  • Ability to work independently and as part of a team.
  • Ability to research/identify and apply appropriate standards of care.
  • Interest in continuous learning and a commitment to staying informed on regulatory changes.
