Waiver Service SupportFacilitator - I/DD

Performs assessments of I/DD Waiver members for the purpose ofdetermining their individualized budgets which are utilized by the member'splanning team to purchase the services necessary to support the individual inthe community.  Assessments are also used to determine re-eligibility intothe program each year.  The Service Support Facilitator must manage acaseload of assigned members to ensure that assessments are performed and dataentry into the CareConnection web system is completed within the prescribedtimelines. 

* Schedule, conduct andappropriately document member assessments within prescribed timelines

* Enter assessment datainto the CareConnection web system

* Participate ininter-rater activities to ensure consistency in the administration of theassessment instruments and adherence to assessment protocols.

* Participate in monthlyI/DD Waiver staff meetings, quarterly all-staff meetings and other requiredtraining and meeting as necessary

* Extensive travel withovernight stays sometime required to fulfill assessments when the designatedSSF if not available

* Assist with monitoringdepartment procedures and quality assurance/improvement activities

* Other duties as assigned


* Bachelor's degree inHuman Services and minimum 2 years' experience with the WV I/DD Waiver programservice members with I/DD

* Working knowledge of theWV I/DD Waiver program, policies and procedures as well as otherprograms/services available to program members.  Medicaid/Public Sectorservice experience, preferred

* Demonstrated knowledge ofperson-centered planning fundamentals

* Working knowledge ofcomputer applications and ability to navigate through multiple screens

* Maintain confidentialitystandards/HIPAA compliance when evaluating all pertinent issues.
